All students entering 7th grade Must have an updated TDAP Booster and All Immunizations up to date.
All Incoming Kindergarteners Must have grade appropriate Immunizations and a physical completed and turned into the health office BEFORE the first day of school.
All transfer students Must have the Report of Health form completed. If your child entered kindergarten or 7th grade beginning in 2016 or after the medical exemption forms are no longer valid. All immunizations MUST be updated before entry to 7th grade and/or Kindergarten.
Please review the attached new California Law Senate Bill 277 that went into effect January 1st
PLEASE KEEP YOUR STUDENT HOME IF:
FEVER OF 100 DEGREES OR MORE, VOMITING/DIARRHEA OR PINK EYE
YOUR CHILD MAY RETURN TO SCHOOL ONCE THEY ARE FEVER FREE WITHOUT USE OF ADVIL/TYLENOL FOR 24 HRS, NO VOMIT/DIARRHEA FOR 24 HRS AND
ON ANTIBIOTIC DROPS FOR 24HRS FOR PINK EYE.
REMEMBER ALL MEDICATION FORMS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT AND PHYSICIAN IN ORDER FOR YOUR CHILD TO RECEIVE ANY MEDICATIONS AT SCHOOL!!!!!!!
